In Washington I don't bill insurance; those sessions are private pay at $250. If I'm not in-network with your plan in California or Florida, the session fee is $250 and I can provide a superbill.
An itemized receipt for sessions you've already paid for, with the diagnostic and procedure codes an insurer needs to process an out-of-network claim. You submit it to your plan yourself. Whether they reimburse you, and how much, is your insurer's decision - a superbill is a receipt, not a guarantee. It's worth calling the number on your card to ask whether your plan has out-of-network outpatient mental health benefits before you count on it.
