Washington Consumer Health Data Privacy Policy

This policy applies to residents of Washington State and is provided under the Washington My Health My Data Act, RCW 19.373. It covers only consumer health data as that term is defined in the Act. My general website privacy practices are described separately in my Privacy Policy, and the health information I keep about people who become clients is governed by HIPAA and described in my Notice of Privacy Practices.
Categories of consumer health data I collect, and why

Categories of consumer health data that is shared
I do not sell consumer health data, and I never will. Selling consumer health data without a signed authorization is prohibited by the Act, and it is not something I would do in any case.
The categories of consumer health data that are shared, and with whom:

Specific affiliates: I have no affiliates. This is a solo practice and no parent, subsidiary, or commonly-controlled entity receives consumer health data.
The third parties that could receive consumer health data are Formspree (form processing), Google (Analytics and Tag Manager), Psychology Today (verification badge script), and Headway (insurance billing and scheduling).
Your rights under RCW 19.373.040
If you are a Washington resident, you have the right to:
- Confirm whether I am collecting, sharing, or selling your consumer health data, and to access that data, including a list of all third parties and affiliates with whom I have shared or sold it, along with an active contact method for each.
- Withdraw consent to the collection and sharing of your consumer health data.
- Delete your consumer health data.
How to exercise these rights
Email me at kamronpoosti@kindredmentalhealth.com with the subject line “Washington Consumer Health Data Request,” and tell me which right you want to exercise. I will respond within 45 days. If I need more time, I will tell you why within that first 45 days and may take up to 45 additional days. There is no charge for this.
If I deny your request
You may appeal by replying to my decision, or by emailing kamronpoosti@kindredmentalhealth.com with the subject line “Appeal.” I will respond in writing within 45 days, explaining the reasons for my decision. If I deny your appeal, I will give you a method to contact the Washington State Attorney General to submit a complaint. You may also contact that office directly at atg.wa.gov/file-complaint.
